Donald, go to the menu: Advanced > Forms > Fields ( you of coarse have to have a field chosen first before going to this menu).
• Create Multiple Copies, will create a multiple set of fields on one page, a table array.
• Duplicate, will create copies of the chosen field(s) across multiple pages.

When you say, "...from five different forms..." are you saying from 5 different PDFs to one PDF? In this particular case the only way to do this is to open one PDF, select the field(s) that you want, copy, go to the PDF you want to paste into and paste when you get there.

Unfortunately Acrobat will not remember where the location of your field was so it will paste it in the center of the page. There is no Paste In Place either. You will need to repeat your copy and paste operation as many times as needed.

Yes, five different PDF's to one PDF. PDF's are the same, just have to transfer data from each one, to a final PDF. Certainly, hope there is another method, rather than Copy and Paste.
