I´ve word97 on W2K and acrobat 5.0. I use acrobat-convert to PDF and NOT the print option.
Since MS Word 2007 you can save PDF file from Word and convert source document bookmarks AND / OR headings to PDF bookmarks. If advanced / Acrobat-specific PDF manipulations are
not required, you can use only MS Word to convert DOC(X) bookmarks or headings to pdf named destinations. But you should use third-party add-ins for MS Word.

PDF Destinator for Word add-in extends MS Word's standard export options and allows to convert PDF bookmarks (or document headings) to PDF named destinations during the PDF export process. You can also validate bookmarks, define custom character replacements, point existing bookmarks to newly created Named Destinations etc.

So, you do not need a full version of Adobe Acrobat any more to create PDF named destinations. More info: http://pdfw.grifir.com
