Mail merging with existing PDF

I have some price lists generated in PDF.

I would like to take one of these PDF files and mail merge it with a
list of customer names & addresses to create individual price letters.

How can this be done? (I'm getting a licenced version of Adobe
Acrobat Professional in the next day or two, so haven't had a chance
to play with it yet).

It can be done in Acrobat using a custom-made script written in
JavaScript.
Alternatively, I've developed a web-service that will allow you to do
that without needing any code, or even Acrobat! You just upload the
PDF and the list, and get back the mail merged files. It will even
automatically email them to the recipients, if you want.
